All forms listed below are available to download & complete. Please bring completed forms to your next visit or email them to info@oasismassag.biz. To help you determine which form(s) you need, we have included a brief description of when each form is required, as well as the purpose of each form. Feel free to call with any questions.
We created the majority of our forms by combining various online resources and tailoring everything to suit our specific needs. Not all forms are needed - or intended - for client use; this is especially true of our Covid Forms. We share all of our back-end forms to help others create similar forms for their own businesses.

Client Forms
- Intake Form: All new clients must complete prior to receiving any services. Clients will be asked to complete a new intake annually.
- Company Policies: All new clients are required to initial & sign before their first visit. This form will only need to be re-signed if we make any changes/updates to our policies.
- Health Status Update: All clients must complete before all appointments. This form allows us to track treatment progress and focus on each client's specific needs for that day.
- Pregnancy Intake: All pregnant clients must complete this as well as the intake form prior to their first appointment. Clients will be required to complete this again if they become high risk.
- Fibromyalgia Initial Questionnaire: We recommend that all clients with fibromyalgia complete this as well as the intake form prior to their first appointment.
- Fibromyalgia Pre-session Intake: We recommend that all clients with fibromyalgia complete this prior to each appointment. (Replaces health status update.)
- Minor Release : Any client under 18 must have a parent/guardian complete this form prior to any services being received. Parent must remain on-site during their child's appointment; please do not drop child off.
- OM Team Membership Contract: Required for clients that wish to sign up for one of our monthly membership programs. (Specify which membership when signing.)
- SOAP Notes: This form is used by our staff to track progress of treatments over time. The completed forms get typed into MoPro (massage industry software; completely offline) for insurance billing purposes. Clients do not need to complete this form.
- Chair Massage: This form is used by our staff during events for liability purposes as well as to collect contact information from interested potential clients. We will bring this with us; you do not need to download.
Insurance Forms
- Massage Prescription: This form needs to be completed for all claim types. Please have your physician complete this to document medical need for massage therapy, as well as provide us with the diagnostic codes we need to bill your insurance company.
- Pain Assessment: This form provides us with specific information related to injury and, combined with the massage prescription, allows us to develop a treatment plan.
- Personal Injury / Auto Accident / Slip & Fall: This form covers the majority of claim types. It details the incident resulting in your claim and provides us with information we need to verify your coverage.
- Work-Related Injury: This form is for Worker's Comp claims only. It details the incident resulting in your claim and provides us with information we need to verify your coverage.
- Release of Records / Payment Agreement / Assignment of Benefits: This form allows us to communicate with applicable parties regarding your claim, and explains financial obligations & terms regarding insurance billing.
- Attorney Letter of Protection: This form is required if your claim involves a lawsuit against another party.
Covid19 Forms
- Covid Informed Consent & Liability Waiver: All clients must initial & sign this prior to their first appointment after reopening (6/30/2020).
- Covid Pre-screening Questionnaire: All clients are required to pass this prior to receiving any services. Client will be asked to sign to attest their answers.
- Phone Script for Pre-Screening Questionnaire: We use this script to determine client's exposure risk prior to appointments.
- Cleaning Logs: These forms outline our sanitation protocols, who is responsible for completing them, and tracks when they have been completed. Logs are kept on the door to the applicable room or on the wall in the applicable area. Completed logs are stored in a dedicated 3-ring binder; logs are archived annually.
