Client Forms

Required forms vary by appointment type. Clients may complete them in person, download PDFs using the links below, or request digital forms via email to complete prior to their appointment.

  • Informed Consent: During your intake, your practitioner will review and complete an informed consent form with you. After a brief verbal discussion, you’ll sign to confirm your understanding and agreement. This one-time form documents the services you consent to receive, the areas you would like treated or avoided, and any relevant precautions or contraindications. It only needs to be updated if your preferences or health information change.

  • Health History: All clients must complete a health history form before receiving any services. This form helps your practitioner provide safe, appropriate care and must be reviewed and updated annually. For paper forms, annual review signatures are included. For digital submissions, a new form is required each year.

  • Massage Therapy Client Intake & Company Policies: This one-time form is required for all clients booking their first massage appointment. It helps us understand your goals, preferences, and relevant health information, and includes contact details, communication preferences, and acknowledgment of our company policies. Clients who are not receiving massage services do not need to complete this form.

  • Personal Training Intake & Company Policies: This one-time form is required for all clients booking their first personal training or fitness appointment. It documents your goals, preferences, relevant background information, and acknowledgment of company policies. Clients who are not receiving personal training services do not need to complete this form.

  • Pregnancy Massage Intake: Pregnant clients are required to complete this form for each pregnancy prior to receiving services. It helps ensure that care is tailored appropriately and safely. A new form is only needed again during the same pregnancy if the client is re-classified as high risk.

  • ​Consent for Treatment for a Minor or Client Who Is Unable to Grant Consent: This form is required before services can be provided for any minor or individual unable to grant legal consent. It must be signed by a parent, legal guardian, or authorized representative. A responsible adult must remain on-site for the duration of the appointment; drop-offs are not permitted.​

  • Referral for Massage Therapy: This form is recommended for clients seeking insurance reimbursement or using an HSA or FSA account to pay for services. Referral requirements vary by provider, and updated documentation may be needed periodically.
