Client Forms
Required forms vary by appointment type. Clients may complete them in person, download PDFs using the links below, or request digital forms via email to complete prior to their appointment.
- Massage Therapy Client Intake & Company Policies: A one-time form required for all clients booking their first massage appointment. This form gathers massage goals and preferences for massage therapy. It also includes contact information, communication preferences, and acknowledgment of company policies. Clients who are not receiving massage services are not required to complete this form.
- Health History: All clients are required to complete this form before receiving any services, and it must be updated annually. The paper form includes additional signature lines for annual review and acknowledgment. For digital submissions, a new form must be completed each year, as the review option is not available for digital forms.
- Personal Training Intake & Company Policies: A one-time form required for all clients booking their first personal training or fitness appointment. This form gathers goals, preferences, and acknowledgment of company policies. Clients who are not receiving personal training services are not required to complete this form.
- Pregnancy Intake: All pregnant clients must complete this form for each pregnancy before receiving any services. A new form is only required for the same pregnancy if the client is classified as high risk.
- Consent for Treatment for a Minor or Client Who Is Unable to Grant Consent: This form is required before services can be provided for any minor or individual unable to grant legal consent, ensuring protection for both the client and the business. A parent, legal guardian, or legal representative must remain on-site for the duration of the appointment--drop-offs are not permitted.
- Referral for Massage Therapy: This form is recommended for clients seeking insurance reimbursement and/or using an HSA or FSA account to pay for services. A new referral will be required periodically, based on insurance or account provider requirements.
