All forms listed below are PDFs available to download & complete. Please bring completed forms to your appointment. To help you determine which form(s) you need, we have included a brief description of when each form is required, as well as the purpose of each form.

Client Forms

• New Client General Information*: One time form that all new clients are required to complete. This form includes contact information, communication preferences, and company policies.
• Health History*: All clients are required to complete this prior to receiving any services; form needs to be updated annually. This form has extra signature lines for client to sign off that they have reviewed it so they don't have to complete it annually. T
• Massage Intake: One time form for all clients at their first massage appointment. This form includes massage goals and preferences as well as informed consent for massage therapy. Clients that are not receiving massage do not need to complete this form.
• Pregnancy Intake*: All pregnant clients must complete this for each pregnancy prior to receiving any services. They will only be required to complete it again for the same pregnancy if they become high risk. 
• ​Consent for Treatment for a Minor or Client Who Is Unable to Grant Consent: This form is required prior to receiving services for any minor or an individual who is unable to grant legal consent.  This protects the client as well as our business. Parent/Legal Guardian/Legal Representative must remain on-site during the appointment; please do not drop off.​ ​
