New Client Intake Forms (15 min)

The required forms vary by appointment type; see descriptions below. Clients may fill them out in person, download pdfs using links below, or complete the digital forms (automatically emailed to new clients) to prior to your appointment.

• New Client General Information: One time form that all new clients are required to complete. This form includes contact information, communication preferences, and company policies.
• Health History: All clients are required to complete this prior to receiving any services; form needs to be updated annually. This form has extra signature lines for client to sign off that they have reviewed it annually. Review option not available for digital form; digital form needs to be completed annually.
• Massage Intake: One time form for all clients when booking their first massage appointment. This form includes massage goals and preferences as well as informed consent for massage therapy. Clients that are not receiving massage do not need to complete this form.
• Lifestyle Questionnaire: One time form for all clients when booking their first personal training session. This form includes fitness goals and preferences as well as liability waiver. Clients that are not participating in personal training do not need to complete this form.
• Pregnancy Intake: All pregnant clients must complete this for each pregnancy prior to receiving any services. They will only be required to complete it again for the same pregnancy if they become high risk. 
• ​Consent for Treatment for a Minor or Client Who Is Unable to Grant Consent: This form is required prior to receiving services for any minor or an individual who is unable to grant legal consent.  This protects the client as well as our business. Parent/Legal Guardian/Legal Representative must remain on-site during the appointment; please do not drop off.​ ​
